WHAT is the Record of 4-H Achievement?
The Record of Achievement is a form to help a 4-H member maintain an accurate account of their 4-H accomplishments
WHY do I need to fill out this form?
This is a way for 4-Hers to keep track of their 4-H Accomplishments. You can utilize the Record of Achievement when applying for scholarships and awards through 4-H.
WHERE do I get one and where is it kept?
The Record of Achievement is available at the Tippecanoe County Extension website at https://www.extension.purdue.edu/tippecanoe. You may download this form and save it to your computer, in order to type directly on the form.
HOW do I fill out the form and keep it updated?
An example is shown on the next two pages. You can either edit this document on a computer or print off a copy and directly write on your Record of Achievement. The Record of Achievement should have a record of 4-H accomplishments only.
4-Hers should begin completing this form their first year of 4-H, updating the White Achievement cards each year showing the projects taken, ribbon placing, grades from their record books, and activities, meetings, etc. is very important.
4-H Achievement Cards are no longer turned into the Extension Office and/or stored at the Extension Office. These forms are the responsibility of the 4-H Member. It is up to each 4-Her to update and manage their Achievement Card each year.
HOW do I apply for County Awards?
The Tippecanoe County Extension Office will reach out over the summer with different award opportunities that 4-Hers can apply for. These award opportunities are listed below. If a 4-Her meets the award criteria and would like to be considered for the award, the 4-Her will need to bring a copy of their Record of Achievement to the Extension Office. The Awards Committee will review all of the Records of Achievement and choose winners. Once the winners are selected, the Records of Achievements will not be returned to the 4-Her and they will be discarded. Award winners will be notified by the Extension Office and invited to the Award Ceremony during the Tippecanoe County Fair.
